How to Claim

In order to make sure that you get fast and efficient service when you  come to claim at any of our branches, please make sure that you have the right documents for what you are coming to claim for.

Death Claim

  • Certified death certificate of the deceased
  • Policy document
  • Certified copy of beneficiary’s Omang if a citizen of Botswana or passport for foreigners 
  • Certified copy of birth certificate where the beneficiary is a minor and doesn’t have an ID card
  • Proof of account; e.g.  Copy of bank statement/cancelled cheque leaf.

Please note that Death claim requirements are also applicable to Waiver of Premiums on payer’s death.

Accidental Death Claim

  • Certified copy of beneficiary’s Omang if a citizen of Botswana or passport for foreigners 
  • Certified copy of birth certificate where the beneficiary is a minor and doesn’t have an ID card
  • Proof of Account; e.g.  Copy of bank statement/cancelled cheque leaf.
  • Certified copy of driver’s license if the deceased was the driver
  • Police and witness report
  • Certified death certificate of the deceased

Family Commuter Cover

  • Certified copy of beneficiary’s Omang if a citizen of Botswana or passport for foreigners 
  • Certified copy of birth certificate where the beneficiary is a minor & doesn’t have an ID card
  • Proof of Account e.g.  Copy of bank statement/cancelled cheque leaf.
  • Certified copy of driver’s license if the deceased was the driver
  • Police and witness report
  • Certified death certificate of the deceased
  • Driver’s license if the insured was the driver

Disability Claim

Sick and annual leave report

  • Form A – Personal Statement & Job Description Questionnaire (completed by the claimant)
  • Form B – Confidential Medical Report (completed by the General Practitioner
  • Form C – Confidential Medical Report (completed by the Specialist Physician)

Disability claim requirements are also applicable to Waiver of Premiums on disability.

Hospital Cash Claim

  • Certified copy of policy owner’s Omang if a citizen of Botswana or passport for foreigners 
  • Certified copy of beneficiary’s National ID if a citizen of Botswana or passport for foreigners (Individual confined to hospitalisation)
  • Hospital discharge summary and doctor’s report
  • Proof  of account; e.g. copy of bank statement/cancelled cheque leaf

Maturity Claims

  • Policy document
  • Certified copy of beneficiary’s Omang if a citizen of Botswana or passport for foreigners
  • Proof  of account; e.g. copy of bank statement/cancelled cheque leaf

Pension Maturities

  • Policy document
  • Proof  of account; e.g. copy of bank statement/cancelled cheque leaf
  • Certified copy of policy owner’s ID/Omang

Pension Maturities (due to ill health)

  • Certified copy of policy owner’s ID
  • Supporting medical information
  • Proof  of account; e.g. copy of bank statement/cancelled cheque leaf
  • Electronic Funds Transfer (EFT) Form (completed by the claimant)

Please note that pension maturities are forwarded to BURS for tax clearance before processing can be initiated.

Cash-back Claims

  • Copy of policy owner’s ID
  • Proof  of account; e.g copy of bank statement/cancelled cheque leaf

Encashment

  • Copy of policy owner’s ID
  • Proof  of account; e.g copy of bank statement/cancelled cheque leaf
